These special needs about me pages are great for sharing important and helpful information with people in your child’s life.

Who could you share this about me form with?

You can share these info sheets with family members, babysitters, occupational therapists, physical therapists, speech therapists, ABA therapists, BCBCA therapists, teachers, employers, and more. Perhaps your kiddo will be seeing a new therapist, starting school with a new teacher, or joining an extra-curricular club or activity. It’s important to be able to share information that will help others best support and care for your child.

Who is this about me printable for?

Whether your child has Autism (ASD), Attention Deficit Hyperactive Disorder (ADHD), a learning disability, Sensory Processing Disorder, Visual Impairment (VI), is deaf or hard of hearing, or requires support in other ways, these pages can provide an organized way to communicate with those who interact with and care for your child.

Details

Each printable includes the following sections:
- Name
- Age
- Birthdate
- An “I have” section to note diagnoses, and/or fun facts, like family members’ information, pets, special skills or talents, assistive technology your child uses regularly, etc.
- Medical info
- I enjoy
- An “I need extra help with” section for areas where your child needs additional support. Some examples include (but are not limited to) transitions, academics, communication, socialization, the environment, self care, emotional regulation, etc.
- A “supports that help me” section that describes ways to help your child be successful based on the “I need extra help with section.” For example, if your child needs extra help transitioning from one task to another, you can make a note for strategies that work for your child, such as a 2 minute warning before switching tasks.
- Sensory needs (including subsections for things your child seeks/enjoys and avoids/has a difficult time with)
- An “other helpful info” section to describe any other details that would be beneficial to a person providing support or care for your child
There are 2 framework options for these about me pages:
- A one-page option that includes all of the about me sections on one sheet
- A two-page option that includes more room in each section to add additional information
There are 2 color options:
- Pink and purple star design
- Blue and yellow star design
Filling out the forms
- These pages are available in PDF files, which you can print and fill out by hand. However, if you are savvy with design, publishing, or word processing software, you may be able to add digital text fields.

Printing The Special Needs About Me Pages

These pages print easily on standard letter size (8.5 x 11 inch) paper.

I recommend printing on heavy duty card stock or matte photo paper for the best durability and color quality. These pages can be laminated as well.
